We know that maintaining your health and well-being sometimes requires a holistic approach, so we’re offering our Network Health Extend members money back on acupuncture services.

How to get your reimbursement

• Visit any licensed acupuncturist.
• Make a copy of your acupuncture receipt; be sure it shows the acupuncturist’s license number.
• Print the Reimbursement Form (PDF) or call us for one at 888-257-1985.
• Fill out the reimbursement form (be sure to make a copy for your records).
• Mail the completed form along with your receipts to:

Please note

• You must be a Network Health Extend member at the time of your appointment and when we process your reimbursement. 
• You should get your reimbursement in 4 – 6 weeks.
• Network Health Extend members can get one reimbursement of up to $150 every 12 months.
