How to get a FREE gift card

• Schedule a yearly checkup with your child's primary care provider.
• Print a Reward Form (PDF) or call us for one at 888-257-1985 (TTY: 888-391-5535), Monday through Friday,
  8 a.m. to 5 p.m. We're happy to help.
• Complete the form and have your child's primary care provider sign the form.
• Keep a copy of the form for your records.
• Mail the completed form to:

Please note

• Your child must be a Network Health Extend member at the time of the visit and when we process your order.
• You should get your gift card in 4 – 6 weeks.
• Network Health Extend members (ages 3 – 19) can get one gift card every 12 months.
