Pharmacy update: Suboxone
Recently, the manufacturer of Suboxone announced that they will soon halt production of Suboxone tablets. In anticipation of this change, Network Health will include Suboxone film on our Preferred Drug List effective October 15, 2012. Please transition your patients using Suboxone tablets to Suboxone film as soon as possible.
We will send all of our providers written notification of the following changes, prior authorization requirements, and quantity limits for Suboxone.
Effective October 15, 2012, we will cover Suboxone film without prior authorization up to the quantity limit of two 8 mg buprenorphine/2 mg naloxone strips per day, or three 2 mg buprenorphine/0.5 mg naloxone strips per day.
- Patients with no recent history of Suboxone tablet use with Network Health — As of December 15, 2012, we will require prior authorization for all prescriptions.
- Patients with recent history of Suboxone tablet use with Network Health — Until February 3, 2013, we will continue to cover Suboxone tablets up to the current quantity limit without prior authorization.
- All Network Health patients — As of February 4, 2013, we will require prior authorization for all prescriptions.
